by the designated deadline will not be considered.

Qualifications & Requirements Checklist

1. Application must be typed or legibly printed in ink.
2. Answer essay questions and self-description appendix.
3. Attach three letters of recommendation (no relatives).
4. Attach the letter of admission from the college you plan to attend.
5. Attach copies of your high school transcripts that indicate your class rank, GPA and ACT and/or SAT scores.
    *** Must be signed by school counselor. GPA of 3.0 or better.
6. Must show proof of at least 20 hours of community service.
7. Attach photo of yourself.

Deadline:
Return the completed application and material by
April 30, 2010 by 4:00pm.

Note: Applications, eligibility requirements and criteria will be reviewed on each application by our Scholarship Committee on or before May 7, 2010.
